A successful filmmaker discovers he has a twin brother, that earns his living as a truck driver. Despite an initial bond, their vastly different lifestyles and unresolved childhood traumas lead to a toxic relationship, culminating in a tragic act of violence.
Joshua Miller, a married truck driver, calls the NSW Central Coast his home.
He feels oppressed by his own sense of inadequacy, which causes him to underachieve. He indulges in his interest in gay kink by regularly attending a sex club/sauna in Brisbane. Whenever he's in the club, he loves the sensation of being pushed to his limits by the skilled dungeon master. His pleasure comes from pain, and he craves it above all else. Occasionally, he catches the scent of a teenage boy, and what starts as a quiet, sensual affair always ends in a brutal murder by strangulation. When he strangles an undercover police officer the pressure builds.
The existence of an unknown twin brother becomes an all-consuming distraction for him. He can't help but feel envious of his twin's charmed life, which only emphasizes his own failures. His brother donates a kidney and it has the opposite effect. He attempts to frame his brother as he continues to increase the body count. As his yearning for his brother’s charmed life escalates, it drives him to either possess or destroy his brother. Even rape and incest become a tool in his twisted mind and the tension build to a crescendo as their relationship crumbles.
